You need to contact the Department of Road Transport in your area or province, you also need some papers from several offices which show that the person is disabled and needs the modifications.
If you can tell where you live I can post the Department of Road Transport contact information

Great to see somebody with a disability not letting it get in the way of enjoying the freedom of motorcycling.
Back home in West Australia, i have a next door neighbour, an old guy ( in his late 50s) named Bill. He has one leg shorter than the other due to a motorcycle racing accident when he was younger.
He and his wife still ride and are members of the Ulyssys Motorcycle Club, he rides a Suzuki Burgman 400 and his wife rides a Aprillia Mana 850 Auto which Bill also rides from time to time when he wants to really "get on the gas"
Its so good to see him out riding and not letting his disability stop him from enjoying what he loves.
The Can-am Spyder 3 wheel bikes and the 3 wheel piaggio bikes might be suitable for some others with a disability.
cheers
Matt.....

From experience I also can tell that the Department of Land Transport is not that difficult for somebody with a genuine disability.
